      1. The Staten Island Sports Medicine Center will be open daily. Appointments should be made by calling (718) 351-6500 between 9 a.m. to 5 p.m. During football season, the office will be open on Saturdays from 4 p.m. to 6 p.m. for acute emergencies only. Coaches must call to tell physicians they have an injured athlete. Do not just walk into the office.
      2. In the event of an acute injury a special appointment will be made so the patient can be seen immediately.
      3. In an emergency, you may use the emergency room at St. Vincent’s Hospital. Please notify the nurse that the athlete is referred by Staten Island Sports Medicine Center and that she is to immediately contact Dr. Mark Sherman, Dr. Joel Bonamo or Dr. Maryirene J. Flynn.
      4. In order to facilitate treatment, please make sure that the parent or legal guardian accompanies the athlete to the center. If this is not possible, a release form should be available, signed by the parent or guardian. It would be a good policy for the coach to have these forms on file at the beginning of the season in case they are needed.
      5. Please note that the doctors are private physicians and are consultants for the PSAL. Please make the parents aware that they are responsible for submitting their insurance forms upon request, or treatment cannot be administered.
      6. The athlete must have a completed PSAL insurance form signed by both the parent and coach when he or she visits the Sports Medicine Center. In case of hospital confinement and/or surgical procedures the athlete must also bring private insurance information along with the Insurance Claim form on the first visit. A patient will be refused treatment if he/she does not have the necessary forms properly filled out. If possible, coaches should have forms at all games.
      7. The Sports Medicine Center will try to provide physicians for games needed. Please call two (2) weeks prior to the game if you are in need of a physician.
