Athletic Insurance Book

The PSAL, as an office of the Department of Education, participates in the City of New York's self-insured retention program which includes coverage for all interscholastic public high school teams representing the Public Schools Athletic League. 

In regard to accidental injury to individual student-athletes, an insurance carrier, currently The Maksin Group administers a program under which all PSAL participants, including coaches, staff members, players and student managers, are insured.

All Principals, Athletic Directors and Coaches are reminded that a separate Insurance Book,  which is considered a legal document, must be actively maintained for every team.  The information in the book must be updated and accurate at all times. It should always be available for review by administrators and auditors. 

For every student who tries out for a team the following information must be maintained:

Last Name

First Name

Date of Parent Consent

Date of Medical

Date of Tryout

Drop Date

Emergency Phone #

Date & Nature of injury

The outside cover of the book should include the school name, sport and seasons covered.

The first page for each season should include the name of the sport and the year

The "Insurance Book" must be a bound notebook or computer generated pages kept in a binder or folder. A separate book must be maintained for each team including each track season (cross country, indoor track and outdoor track). The book should be monitored by the Athletic Director and constantly updated by Coaches. It should be kept in the Athletic Director's files and copies of the current seasons' information must be given to each Coach. Coaches should have these copies with them at all games and practices. Insurance Books must be retained in school files for the period of time designated by Chancellor's Regulation.
